Completing the employee case questionnaire

A case questionnaire is generated for each case opened in the Envoy Global platform to ensure the legal team has the necessary preliminary information to begin preparing the case. Information will be requested from the employee and other parties, including the sponsoring employer, if applicable. The case questionnaire must be completed to 100% and submitted to the legal team's review before they can begin preparing the case. 

đź’ˇ For the best experience, complete the case questionnaire using a desktop browser. The questionnaire is not currently supported on the Envoy mobile app. 

We ask that employees complete the questionnaire to the best of their ability, and upload as many documents which apply to the case. The legal team uses the questionnaire to gather preliminary information to begin preparing the case. If they have questions on the information provided or need additional information, employees will be notified.

💡 Look for the “i” information icon to learn more about how to answer the question. If employees still require support to answer the question, contact Envoy Customer Experience.

Help text provided in the “i” information icon will provide additional details on how to answer the question or where to find a document. If the legal team has any questions after their initial review, the employee will be notified.

Questions that are not applicable

Employees can enter “Not Applicable” if there is a question that does not apply to their situation. If a field contains text restrictions, such as a date or numerical value, please enter null information.

The legal team will review any information indicated with null information and will contact you directly if they have any questions. 

Submit the case to the legal team’s review

Once all information is provided to 100%, then the employee will see the option to submit the case to the legal team's review. However, if action is needed from another party, then the option will not appear.
